These are a set of forms to print and fill out prior to your first session if you have a printer.

Due to the current
economy and the
impact it is having
on people,
counseling may be
more important
now than ever. For
this reason, I am
presently willing to
lower my fees
further than usual
for those needing
to work on a
sliding scale.

Fees and Availability

Hours:  Generally, I am available Tuesday through Fridays, 12 noon  - 7 pm.

Fees:  My base rate is $125 per 50 minute hour and $180 for 75 minutes. For those
paying out of pocket (not using insurance), discounts up to 20% may be available. Often,
people prefer to pay for a longer period of time for their first visit and it is standard for
insurance companies to allow for a longer initial session. Although I do not offer free first
sessions, I am available prior to our first session by phone or email at no charge.

Insurance:  My standard fee is $125 per hour. Payment is required at the time of service
in the form of cash or check.

If you are using insurance, I will show you how to fill out the forms for your reimbursement.
Because insurance policies tend to be so different from one another, I advise you to
contact you insurance company prior to making your first appointment in order to make
sure you know what services are covered. I have prepared a list of questions for you to
ask them which can be found on my
Insurance Information Page. To the left are some
insurance companies I have worked with in the past. I am a preferred provider for Blue
Cross/ Blue Shield.

Sliding Scale:  My sliding scale rarely goes below $100 per hour, but during these times of
economic hardship, I may be willing to negotiate a slightly lower fee.
